Exploration du vécu d'un programme d'éducation thérapeutique et des représentations du diabète gestationnel : étude qualitative réalisée en 2015 auprès de huit femmes ayant participé au programme mis en oeuvre à la maison de santé pluridisciplinaire « Artemis » à Saint Denis de la Réunion

Abstract : Gestational diabetes is a common pregnancy complication which prevalence in La Réunion was about 14 % in 2013. On this island, the regional policy is oriented towards the development of Therapeutic Patient Education (TPE) in the ambulatory care sector. We asked ourselves what experiences people who participated in such a programme had gone through. We therefore conducted a qualitative study with semi-structured interviews with date saturation of women who participated in the « Artemis » programme on gestational diabetes. Eight women were interviewed in Saint Denis De La Réunion between August and October 2015. This study also aimed to determine the role and importance of the general practitioner in such care. The main results first show that gestational diabetes is experienced by The Réunion women as a disappointing or surprising event, tiring, restrictive and terribly anguishing. Then the therapeutical education programme is perceived as real support. Indeed, women felt supported, assisted, supervised and never judged. They appreciated a care adapted to who they were, their experiences and their environment. Finally the general practitioner (GP) is described as missing when it comes to those complicated pregnancies. It apears that he GP knows the participants, he listens to them, informs them, gives them advice, follow them and guide them but the role of prevention is unnoticed. We propose, in the perspective of improving the programme, to provide additional information in the shared educational diagnosis, to quantify the objectives for further evaluation and to personalise all mail to the doctor in charge. To conclude, the risk being to develop type 2 diabetes, the main challenge is to continue and maintain the support given to those obese women. This could be achieved through better collaboration between health professionals and through the developed health networks on the island.
