Analyse des difficultés rencontrées lors de la reprise du travail après un cancer : spécificités du cancer du sein

Abstract : Introduction: Life after cancer is one of the major issues of the 2nd "Cancer Plan" in France. In this context, our study aims at analyzing professional consequences of breast cancer, which has a relatively good prognosis and is the most common cancer in the working population. Methods: Medical and professional information about 402 workers from the Paris region who had cancer in 2005/2006 was collected by occupational physicians using a self-administered questionnaire. Multifactorial analysis defined a pattern of difficulties faced by workers. Then, focusing on the 145 cases of breast cancer, we searched for predictive factors of these difficulties and specific aspects of breast cancer compared to other cancers using comparative analysis. Results: 84% of women with breast cancer returned to work and median duration of the sick leave was 10 months. Only 22% met the occupational physician to plan returning to work while they were still on sick leave. After returning, 50% faced difficulties at work, half of which felt penalized. Predictive factors of a problematic re-entry at work included: not feeling accepted by others and feeling uncomfortable with one's social life (p<0,05). Chemotherapy, a long sick leave, or socio-professional group showed only non significant results probably due to a lack of statistical power. Compared to other cancers, breast cancer was more frequently associated with chemotherapy, a longer sick leave, movement impairment, memory disorders or impaired concentration. Conclusion: Our study shows that more than half the women with breast cancer experience serious difficulties when returning to work. Factors that may predict such difficulties must be known in order to better anticipate and improve return to work after breast cancer. We also believe that enhanced communication between attending and occupational physicians, and better information of all actors, including patients and their workplace is needed.
