Reconnaissance des arrêts cardio-respiratoires par les opérateurs du centre de traitement des alertes du Service Départemental d'Incendie et de Secours 64-Pyrénées Atlantiques

Abstract : Introduction:The recognition of out of hospital cardiac arrests is the first step in the chain of survival and earliness is necessary. It must be performed as soon as the emergency call has been received because it allows immediate cardio pulmonary resuscitation, cardiac defibrillation and implementation of emergency resources. Equipment and method: Conducting a prospective analysis of the practices of SDIS 64 Alert Treatment Centre dispatchers in their cardiac arrest recognition mission, listening call recording tapes for unresponsiveness calls. Results:107 calls were analyzed of which 17 were cardiac arrests on arrival of the rescue team. Cardiac arrests recognition rate was 23.5%. The sensitivity of the interrogation was 25.5% and the specificity was 98.9%. 11 of 13 cardiac arrests undetected were identified as breathing normally. Checking up unresponsiveness was made for 83.2% of calls. Seeking evidence of breathing was made for 66.2% of those estimated unresponsive and looking for breathing quality was made for 2.1% of those deemed breathing. Discussion:The level of cardiac arrests recognition efficiency can be improved. Misidentifying respiratory status is responsible for most of the unrecognized cardiac arrests. Search breathing quality is almost never performed, probably due to poor knowledge of agonal breathing. Implementation of an IT protocol support for recognition of cardiac arrest coupled with specific training in identifying of agonal breaths on the phone can be offered to dispatchers.
