Vitamin k antagonists : pharmaceutical consultations, advices and patient care at the dispensary

Résumé
Vitamin K antagonists are the first cause of iatrogenic accidents in France. These narrow therapeutic range drugs need a reinforced surveillance and a good knowledge of the treatment. Hemorrhagic complications linked to an overdose or thromboembolic problems due to under dose can be avoided. In a public health role, the pharmacist is now allowed to make pharmaceutical consultations in order to support patients with a chronical treatment by vitamin K antagonists. He can complete the physician’s role in the follow up of this treatment, to improve observation and prevent potentially lethal adverse reactions caused by the wrong use of these drugs. The hemostasis physiology and the pharmacology of the oral anticoagulants are presented in the first and second part of this manuscript. The last part is dedicated to the pharmaceutical consultations, completed with an opinion survey.
Les antivitamines K constituent la première cause d’accidents iatrogéniques en France. Ces médicaments à marge thérapeutique étroite nécessitent une surveillance renforcée, et une bonne connaissance du traitement, afin d’éviter les complications hémorragiques d’un surdosage, ou thromboemboliques d’un sous dosage. Dans un rôle de santé publique, le pharmacien peut désormais faire des entretiens pharmaceutiques afin d’accompagner les patients ayant un traitement chronique par antivitamines K. Il vient se placer en complément du médecin dans le suivi de ce traitement, pour en améliorer l’observance et prévenir les effets indésirables potentiellement fatals d’un mésusage de ces médicaments. La physiologie de l’hémostase et la pharmacologie des AVK sont présentés en première et seconde partie de cette thèse. La dernière partie est consacrée aux entretiens pharmaceutiques complétée d’une! enquête d’opinion.
