Les facteurs favorisant le stockage des médicaments : analyse descriptive et étude qualitative au domicile de patients de plus de 65 ans

Abstract : Introduction: In France, medicine holds an important place in public health expenses. Its consumption has been estimated to 48 pill boxes a year per person. Prescription drugs also have one of Europe’s highest average selling cost. There is an important storage and waste rate in private medicine cabinets. The main purpose here is to research for the factors favoring the storage and waste, in the place of residence of over 65 years-old patients. Methods: A descriptive analysis of private medicine cabinets content has been led from September 2014 to September 2015, among the patient base of nurse practicing in an urban environment, and the patient base of 3 General Practitioners set in rural and semi-rural environments. It has included 40 households of people aged 65 and over. The qualitative study by semi-directed interviews occured in February 2016 and has included 11 on the 40 households observed in the descriptive study. Resu! lts: 40 households, 55 people, took part in the descriptive study, and represent an average age of 81,14 years-old. This households consisted of an average of 1,43 people, 67,3% being women and 32,7% being men. We have found an average of 49,3 pill boxes per home, for an average cost of 405,2 euros; 13,5% were expired, 23,6% were discontinued treatments and 56,9% were unused (expired or discontinued). There was a correlation between medicine storage and the high number of chronic diseases, the absence of a third, or, on the contrary, the multiplicity of stakeholders in the drugs management, as well as the consultation in the medical office and a high number of storage places. From the patients point of view, the factors favoring medicine storage were an unadapted pill number packaging ; poor compliance and a lack of trust in medicines and generic medicines. Conclusion: Our study shows a consequent private medicine storage and an important waste rate. Factors favoring storag! e show that an effort has to be made on packaging and on the w! ay patie nts are informed about managing their own medicine cabinets, while also improving coordination between stakeholders, health professionals and patients.
