Impact du délai de déclenchement chez les patientes présentant une rupture prématurée des membranes à terme avec un prélèvement vaginal négatif

Abstract : Introduction : The premature rupture of the fœtal membranes occurs in 6-10% of women with pregnancies at term. In the absence of sponteneous onset of labour within 48 hours, there is a risk of infection which it is necessary to induce labour. However, the fear of an increasing rate of cesarean section ceases some serious doubt as to this method of childbirth. The aim of this study was to compare the impact of two times periods of labour induction (within the span of 36 hours) on the rate of cesarean section and on the rate of maternal and fetal infection. Materials and methods : An observational and retrospective study included 237 women of the maternity unit of Bordeaux was carried out over a period of time from 1st January to 31st December 2014. We compared two groups of women with non obstetrics-risk factors and premature rupture of membranes at term with negative maternal group B streptococcal status. A third analyzed group ! consisted of a sample of women with spontaneous labour. Results : There was no significant difference of the rate of cesarean among women where the labor was induced within the span of 36 hours (8,8% versus 9,7%, p= 0,90). Also no difference was reported on the rate of maternal or fetal infection (32,4% versus 19,4%, p= 0,40). 90,9% of women got a spontaneous labor within 25 hours. Modes of delivery were the same among the women with an inducing labor. No statistical difference was observed in this group in terms of maternal or fœtal infections (22% versus 17%, p= 0,78%). Discussion and conclusion : Both the rate of cesarean as well as maternal and fetal infection are the same if the labor is induced within the span of 36 hours. It is also suggested to induce the labour on maternal request, provided the favorable cervix conditions.
