Impact de l’Indice de Masse Corporelle maternel sur la survenue de déchirures périnéales sévères : étude cas témoin au CHU Grenoble Alpes sur 376 dossiers

Abstract : Objectives: Severe perineal lacerations represent a significant complication of vaginal delivery. Prevalence of obesity among women in childbearing age increases. The aim of this study was to identify the link between maternal BMI and the occurrence of severe perineal lacerations. Materials and methods: A retrospective study was conducted in a case-control group, women with severe perineal laceration (n=124) were compared to those without it (n=376). We compared different classes of Body Mass Index and other maternal characteristics (age, parity), obstetric (gestational age, induction of labour, assisted delivery, epidural anesthesia, episiotomy, operator) and fetal (birthweight) characteristics between two groups. Results: Proportion’s repartition of different BMI classes were not statistically different between the two groups. Other characteristics such as primiparity (p<0,0001), assisted extraction (p<0,0001), occipito-posterior position (p=0,0115), episiotomy (p<0,0001), epidural anesthesia (p=0,0004), maternal age (p=0.0125), operator (p<0,0001) and birthweight (p<0,0001) are statistically associated with occurrence of severe perineal lacerations. Conclusion: This study cannot conclude to an evidence for an association between maternal BMI and occurrence of severe perineal lacerations. This study confirms previous findings concerning severe perineal risk factors.
