État des lieux des pratiques et connaissances sur l'allaitement et la diversification alimentaire à Mayotte

Abstract : The promotion of breastfeeding in response to the alarming nutritional status of children in developing countries is a public health priority. The prevalence of acute malnutrition was 7.3% in Mayotte. The main objective of our study was the description of nursing practice and diversification as well as assessing the level of knowledge of the parents. The secondary objective was to determine predictive factors of malnutrition. Methods: A descriptive cross-sectional study was conducted in the Mayotte health facilities from June to October 2013. The sample selection was done with all infants 0-4 months and from anthropometric measurements (weight, height) of children aged 4 months to 4 years. Nutritional status was assessed using the tables of the World Health Organization and the index Waterlow. Sociodemographic data and criteria for assessment of knowledge was collected by hetero questionnaire. Results: 56.1% of mothers practicing breastfeeding on leaving the maternity, 43.33% at 1 month and 27.78% at 6 months. The milk substitute was introduced on average one month. 20.93% of children over 4 months were weaned and 90.7% were diversified. Knowledge of breastfeeding was correct for 50.7% of the parents. Knowledge of diversification was insufficient to 40.48% of parents. 34,88% of children aged 4 months to 4 years were malnourished. 15.85% of households were in severe precarious situation. Discussion: The results of this survey suggest that breastfeeding is less practiced in Mayotte. The nutritional status of children under 4 years is dependent on the level of knowledge of their parents in nursing and dietary diversification. The precariousness of households plays a role in childhood acute malnutrition.
