Étude quantitative du mésusage de la prescription différée d'antibiotiques dans les infections respiratoires en Médecine Générale

Abstract : Background: Delayed antibiotic prescription (DAP) consists in giving to the patient a prescription with the order to start the antibiotherapy after a given period, depending on symptoms evolution. This type of prescription exists in general practice, but is not studied much in France. Objective: The main goal of the study was to quantify DAP misuse in respiratory tract infections. A misuse was an antibiotic consumption before the given period and/or for a different duration from the prescribed one. The secondary goal was to assess potential variables that may affect it. Methods: A quantitative, observational, descriptive and transversal study was undertaken between January and April 2016. All patients, without any age or comorbidity criteria, being seen by a General Practitioner for an upper or lower respiratory tract infection, for which they had been given a DAP, were included. Data were collected from phone surveys of patients within three weeks after consultation. Results: 88 patients, 6 of whom have been long-lost, were included by 29 doctors. Among the 82 usable surveys, 15 misuses were identified, representing 18,3% (CI 95%, [9,9%-26,7%]): 10 due to consumption before the period end, 5 due to no-respect of the duration. A statistical significant difference was found in patients who were expecting to be prescribed an antibiotic. Conclusion: This result tends to show a satisfactory observance of doctor instructions. DAP is probably not an optimal solution but provides advantages. Patients expecting to be prescribed an antibiotic seem to be the most at risk to misuse. A larger quantitative study could be interesting to precisely know the prevalence of this practice.
