Décès des patients admis aux urgences : étude rétrospective mono-centrique aux urgences du CHU de Pellegrin

Abstract : Introduction: The mission of the Emergency Departments is to welcome every patient. Some of them are going to die of a medical or surgical acute pathology, others of the development of their chronic pathology. Many of them in the final days of their lives are admitted through this privileged way. Few studies have described the epidemiology of these patients. The main objective here is to identify the main characteristics of the patients dying in the ER within 7 days following their admission. Material and method: This is an analytical, retrospective and monocentric study of all the deaths that have occurred over the course of one year, from January to December 2012, of patients having entered the Emergency Department of the Pellegrin Hospital in Bordeaux within 7 days following their admission. Results: 54131 patients have been admitted in the ER of the Pellegrin Hospital over this period. 533 patients were included in this study, representing 0.98% of the work in the department. The average age is 71±19 years old. The main reasons for admission are neurology (40%) and traumatology (22%). 58.3% of the deaths concern patients over 75 years old, the main cause being neurological. For patients under 50, the main cause of death remains traumatology. Among these, 92 died in the ER, representing 0.17%. The average age is 83. 90 patients die within 3 days, among which 48 within 24 hours. 15% of these patients are initially admitted in the resuscitation room. Conclusion: The impact of the deaths in the ER of the CHU Pellegrin in Bordeaux is similar to the data we find of such studies in other French literature. During the admission process in the ER, some of these patients who find themselves at the end of their lives undergo inappropriate intensive treatments, contrary to the patients in palliative care. The time that a patient spends in the ER is influenced by the pathologies and the age of the patients. The percentage of deaths in the ER is low but the conditions there are unacceptable for the dying patients and their families. This situation is a reality we face in the current structure of our job. Several changes will be necessary in order for us to carry out the responsibility to help them pass away peacefully.
