Diagnostic accuracy of ShearWave Elastography compared to Transient Elastography for the non-invasive assessment of liver fibrosis in chronic hepatitis B or C

Abstract : Purpose. To assess the diagnostic accuracy of ShearWave Elastography (SWE) for significant fibrosis (METAVIR score F≥2) in chronic hepatitis B or C, and to compare its performance with Transient Elastography (TE), using liver biopsy (LB) as the reference. Materiel and methods. This single-center prospective study took place from January 2012 to January 2014. For each patient, 10 TE measurements were performed using Fibroscan® (M probe) and 3 SWE measurements using the ultrasound system Aixplorer® in the right liver, followed by LB (fibrosis was staged using the METAVIR scoring system). Results. 90 patients with chronic hepatitis B or C were included. There were 6 patients F0, 46 F1, 25 F2, 10 F3 and 3 F4. For the diagnosis of significant fibrosis (F≥2) and severe fibrosis (F≥3), areas under the ROC curves were respectively: 0.79 [95% confidence interval (95% CI): 0.68-0.90] for SWE and 0.76 [95% CI: 0.65-0.87] for TE (p=0.68), and 0.88 [95% CI: 0.73-1] for SWE and 0.79 [95% CI: 0.59-1] for TE (p=0.07). The failure rate was similar with both methods (16% with TE, 12% with SWE; p= 0.67). Higher subcutaneous thickness and older age were significantly associated with failure for both methods. Conclusion. SWE showed no difference with TE for the diagnosis of significant fibrosis, and their overall applicability were similar. Thus, SWE have the added advantage of being easy to use in daily radiology practice.
