Taux d’hospitalisations non programmées un an après une Évaluation Gérontologique Standardisée en Hôpital de Jour : une étude rétrospective

Abstract : Background: Unplanned hospitalizations increase the risk of complications in the elderly and represent a cost for the society. Comprehensive Geriatric Assessment (CGA) manages to identificate high-risk patients with the aim of establishing a coordinated care plan. Objective: The aim of this study is to determine the unplanned hospitalizations rates after CGA to establish predictive factors of these hospitalizations. Methods: The study was retrospective observational. It was conducted at the day hospital of the University Hospital of Bordeaux. Patients that went through their first CGA in 2014 were included. Rates and causes of admissions were studied. Results were obtained using DxCare software and after calling the patients' general practitioners. Results: 308 patients were included in the study. 58 patients were lost to follow-up. The average age was 85 ± 5 years, with 65 % of women. 75% of the patients lived at home. The unplanned hospitalizations rate was 29 %. This study did not identify any risk factors for unplanned hospitalizations after CGA. However, it appears that a higher MMSE score was associated with a lower probability of unplanned hospitalization. Falls were the leading cause of hospital admissions and represented 34% of all causes. Conclusion: Our work did not highlight any specific risk factor for unplanned hospital admissions after day hospital-conducted CGA. Preventing these hospitalizations should thus be based on a global overview, with no privileged criteria. The CGA performed in day hospital offers multidisciplinary approach that seems suited to the problem of unplanned hospitalization for the elderly.
