Enfance en danger : les freins aux signalements et aux informations préoccupantes chez les psychiatres : étude a partir d’un échantillon de psychiatres en Aquitaine, accueillant des enfants

Abstract : Introduction. Suspected child maltreatment are under-reported to child protective services. Clinicians have a professional position which allows them to be among the first to be able to observe suspected abuse but would present a small proportion behind the reports. It seemed important to us to determine the barriers to these reports, focusing particularly on psychiatrists. Methods. This qualitative study based on semi-structured interviews, included psychiatrists intervened in the monitoring of a minor before it is subjected of reporting in 2013 or 2014, during his hospitalization in child psychiatry in Bordeaux. The objective was to assess the place given to reporting of child abuse among psychiatrists in terms of knowledge, application and especially implementation difficulties. Results. We included 15 psychiatrists. The study shows the obstacles to reporting related to the care, the child, his family, child protection services, the procedure, the suspected facts and the practitioner. Psychiatrists consider their initial education insufficient and express their interest for a multidisciplinary training. Multidisciplinary teamwork and networking, especially for liberal psychiatrists, as well as partnerships between child protection services and the medical community, seemed also necessary. Conclusion. This is, to date and to our knowledge, the first study in France on psychiatrists’ experiences and feelings towards child abuse reporting. This work is in line with the concerns of the French Republic, through the law of 14 March 2016, to "improve national and local governance for Child Protection's". It opens effective training opportunities on support to measuring benefits and risks to undertake such steps.
