Le cancer broncho-pulmonaire dans le département de La Réunion : caractéristiques cliniques et démographiques

Abstract : Primary lung cancer is a common disease primarily linked to environmental factors, but also genetics. In the department of Reunion Island, the population of patients affected by this pathology hasn’t been specifically studied. This work sought to identify all incident cases over a period of three years in order to analyze the characteristics of these patients and to compare them nationally. To achieve this objective, we identified the affected patients and retrospectively collected information on several demographic and clinical characteristics of each patient. In this study, incident cases of lung cancer in Reunion Island were analyzed, from the multidisciplinary meeting data from 1 January 2013 to 31 December 2015. A total of 708 incident cases of primary bronchial cancers was identified. The age-standardized rate (ASR) is 21.3/100,000 person-years. The ASR is 10.7 for women and 33.9 in men. This incidence is among the lowest of the French departments. In women an increase of 2% per year of the ASR has been observed, while there is a stagnation in men. The average age at diagnosis is 64 years with an incidence peak between 70 and 74 years. The diagnosis is usually made at an advanced stage, with 69.2% of patients diagnosed at stage ≥ IIIB. The general condition remains preserved in the majority of patients (79.4% with WHO score 0 or 1). The number of non-smokers is relatively high and about 17.4% of patients are concerned. The adenocarcinomas rate is 62.7% versus 21.5% for squamous cell carcinomas. Major molecular alterations! are related as 24.0% for KRAS mutation, 22.4% for EGFR mutation and 4.5% for ALK rearrangement. These results highlight an ASR among the lowest in France, associated with a much higher adenocarcinomas rate. Also, the rate of patients with access to targeted treatment is greater. This study helps to better know the population of patients with primary lung cancer in Reunion Island in order to optimize the detection and the management of these patients.
