Dimorphisme sexuel de la mandibule : analyse métrique et par morphométrie géométrique, application en chirurgie de féminisation faciale

Abstract : The goal was to analyse sexual dimorphism of the mandible using a metric analysis and a geometric morphometric study in order to provide an anatomical support for facial feminization surgery of the lower third. The sample is based on 60 CT scans (30 males and 30 females) with tridimensional reconstructions. Eight linear measurements and one angle were collected for the metric analysis. Geometric morphometrics with Procruste superimposition was used to investigate sex differences in mandible shapes. The mandibles were traced with 354 landmarks on inner and outer faces of the mandibles. The symphysial width was higher in males than in females but the difference wasn’t significant, whereas symphysial height was significantly different between males and females (p<0,001). The angle between the basilar edge of the horizontal branch and Francfort plane was larger in women ( p=0,01), wich means that the obliquity is more important in women. The geometric morphometric study showed a sexual dimorphism on the condyles and angles giving a triangular shape with ascending branches in women, whereas it was squarer in men. There was also a difference on the chin : the symphysis was less wide in women with a more triangular shape. This study has allowed an accurate analysis of sexual dimorphism of the mandible in 3D using two methods of analysis. The results on the differences in the symphysis were consistent with previous studies. However geometric morphometric analysis showed a difference of inclination from angles relative to the condyles between male and female, changing the shape of the lower third: triangular in women and square in men. This concept had not yet been described. The results will allow us to adapt, case by case, our mandible feminization techniques.
