Évaluation de l'efficacité du bain d'eau chaude isolé dans le traitement des envenimations par piqûre de poisson osseux en France Métropolitaine : étude de 184 cas au cours de l'été 2015

Abstract : Objectives: Weever sting (Trachinus Draco and Echiichtys vipera) and scorpion sting (genus Scorpaena) envenomations are frequent and mild but responsible for an intense pain. The treatment recommended consists of a variation in local temperature with heat then ice. But it’s difficult to find ice on coasts during summer. The treatment of pain whith exclusive hot water bath has never been evaluated. The objective of our work was evaluate the efficiency of exclusive hot water bath in pain with envenomation by weever and scorpion fish. Method : A prospective study was conducted between July, 1st and August, 31, 2015, in 10 lifeguards stations of the National Sea Rescue Society, located in different places on the french coasts. Were included all the patients over 6 years old with weever or scorpion fish envenomation. Upon their arrival (T0), these patients received an evaluation of pain by a Visual Analogue Scale (VAS). Then, the ! affected limb was immersed in a water bath as hot as possible for 15 minutes, then a second evaluation of pain just after the bath (T15), and a last VAS, 15 min later (T30). Children under 6 years old, or patients treated with pain killers 24 hours preceding the sting, or multiple stings victim were excluded. Results : 184 patients were included. The average delay of treatment was of 10,53 minutes after sting. The sex ratio was 1,24, the average age 26,87 years old. The average VAS at the arrival were 5,91. The average water bath temperature was 40,57°C ; its median duration is 14,50 minutes long. The second VAS were 2,06, and the last VAS were 1,04. The difference of VAS is statistically significant (p.value <2,2.10-16). Conclusion : The treatment by exclusive hot water bath significantly reduces pain related to envenomations by weever or scorpion fish stings.
