Outbreak of human metapneumovirus in a French nursing home, a retrospective study

Résumé : Identified in 2001 in the Nederland, human metapneumovirus (hMPV) is an enveloped single stranded RNA virus, with a worldwide distribution, predominantly affecting young, elderly and immunocompromised patients. This thesis describes a hMPV outbreak in a nursing home (NH) in the French administrative area of Isère. The author also aims at identifying risk factors associated to the clinical infection. A retrospective, case-control study was conducted, including all 78 residents of the NH during the outbreak. Nose swabs from symptomatic residents were tested by multiplex RT-PCR in order to confirm hMPV infection as well as to detect coinfections. The outbreak occurred from January 5th to February 5th 2015. A total of 40 residents were infected, corresponding to an incidence rate of 51%. In multivariate analysis, influenza vaccination (OR = 7.58; CI= 1.25 – 45.91; p= 0.028) was independently associated with hMPV infection. Interaction between vaccination and respiratory condition was tested, no significant interaction was found. Finally, the attack rate was high, despite rapid droplet precautions. Unfavorable clinical course in some patients reminds us the necessity of considering metapneumovirus in case of respiratory infection in elderly.
