Évaluation prospective de l’outil informatique Diascope® pour l’aide à l’adaptation thérapeutique du diabète de type 2 en médecine générale

Abstract : Diabetes mellitus is a common disease. Its management is complex but it is a major public health issue. DiaScope® is a computer-aided decision-making tool for the treatment of type 2 diabetes mellitus. The goal of this study was to measure the gap between the practices of general practitioners and DiaScope® proposals for the prescription of antidiabetic treatment in patients with type 2 diabetes mellitus. An observational, descriptive, cross-sectional study was conducted in Grenoble area from May to October 2016. Diabetic patients with insufficient glycaemic control under metformin mono/dual therapy or intolerant to metformin were recruited through the Oriade® laboratories. Medical data were collected from the physicians and biological data from the laboratories. Sixty-seven patients were included. The proposals of the general practitioners were discordant with the recommendations of DiaScope® for 59 patients (88%). Eight patients (12%) had their treatment modified by the general practitioner as recommended by the software. Among the 59 discrepancies, 58 (87%) corresponded to therapeutic inertia and 1 to a therapeutic modification not recommended by the experts. The main reason for inertia recalled by physicians was a low deviation from the glycated haemoglobin target. The study highlight a significant gap between the practices of general practitioners and the proposals of DiaScope®. Discrepancies are mainly due to inertia of therapeutic intensification. The software could help limit this inertia. However, the modifications carried out by doctors match, for most of them, the recommendations of DiaScope®.
