Évaluation de la prise en charge de la gastro-entérite aiguë de l’enfant de moins de six ans en médecine générale : étude prospective menée en Gironde de janvier à avril 2016

Abstract : Introduction: generally benign, gastroenteritis is one of the first purposes for pediatric consultations according to Sentinelles. In 2013 - 2014, children aged from 1 to 4 years old were the most affected (incidence of 10312 out of 100000 inhabitants). The rate of hospitalization was 0,4%. Acute diarrhea can entail dehydration and be a threat to vital prognosis. The loss of weight is the main evaluation factor of dehydration. The work of a general practitioner may be difficult and far from the recommendations because of practical difficulties. The purpose is to describe the practicing of the GPs in the Gironde area on the care of gastroenteritis of children under the age of 6, and the identification of the difficulties met with SRO. Method: the study was based on standardized forecast questionnaires, gived to practitioners in the Gironde area. Results: a sample of 34 practitioners included 143 children whose average age was 38 months between January 1st and April 30th 2016. 9 of them were sent to hospital whereas the others had outpatient care. The figures concerning the stools and vomit are the least checked: 46 and 41% of the cases. The other figures were searched for in 100% of the cases. The initial directing of the patients respected the recommendations in 90% of the cases. More than 90% of the parents are given instructions of supervision and dietetics, however, they are incomplete. Every item appears only in about 50% of the cases. Parents are given an advice sheet, only in 12% of the cases. Practitioners consider that S R O alone is ineffective in 47% of the cases, and they think it is a bad administration in 56% of the cases. As a matter of fact, 22% of the parents did not give the SRO prescribed, and only 58% of them find it effective. Conclusion: things still have to change, to use fewer medics and more explanations. An explanatory document could reduce the pratical difficulties, and also parents anxiety. Bur for the time being, the whole of this approach is considered as being too time-consuming.
