Asymptomatic carriers presenting a high-risk antiphospholipid antibody profile

Résumé

Objectives: To assess risk factors for thrombotic events or obstetrical complications in a targeted cohort of triple positive patients for antiphospholipid tests, a novel score is proposed to better stratify this clinical risk and a short follow up was performed. Methods: Recruitment criteria were an age ranged between 18–80 years old and triple positivity for aPLs tests, namely lupus anticoagulant; IgG/IgM anti-beta-2-glycoprotein I antibody (aB2GPI) and IgG/IgM anti-cardiolipin antibody (aCL). Demographic, laboratory and clinical data were retrospectively collected. Results: Over a 2-year period, 88 subjects were selected and formed two groups according to their clinical status: clinical group and carriers group. A high prevalence of triple positive carriers is highlighted; moreover, having two or more cardiovascular risk factors is an independent risk factor for clinical events. We also evidenced that the double-double profile (which associate double IgG isotype for both aCL and aB2GPI, normalized ratio > 1.2 and Rosner Index > 15) and a Rosner index above 27 are also independent risk factors, and the combination of a high Rosner Index (value exceeding 27) and double-double profile is an important and an independent risk factor for thrombosis or obstetrical complications. All these results led us to propose the Score for Triple Antiphospholipid Positive Patients (STAPP) considering clinical and laboratory data significantly associated with clinical events. Conclusions: Interpretation of aPLs triple positivity needs consideration of clinical and laboratory data. The use of our score, the STAPP, may help clinicians to stratify the clinical risk and initiate or not a primary prophylaxis.
