Morbidité pédiatrique d'un village amérindien en Guyane : étude descriptive chez les enfants de moins de 5 ans à Camopi

Abstract : Purpose: The amerindian village of Camopi in French Guiana is particular due to its geographical isolation and its socio-cultural environment. The children are affected by classic pathologies but also by others due to this particular context. This study aimed at describing the morbidity of children under the age of 5. Materials and Methods: The population sample included residents and children born between January 1st 2009 and December 31st 2013. Sociodemographic, birth, vaccination, and five years medical follow-up data have been collected from health record. The disease incidence has been calculated based on the diagnosis report at the health center. Results: In total, 149 children have consulted 5915 times at the health center, making it to be an average of 9 consultation-year per child. BCG, DTP, HBV and Yellow fever immunization coverage exceeds 95%. Only 4 children are up to date with pneumococcal vaccine. Ear, Nose, Throat and upper respiratory diseases are the most frequent with the incidence of 4,31 cases/ person year. Lower respiratory infections are the most serious diseases since 37,9% of hospitalizations are due to respiratory distress syndrome. Digestive infections are also worrying. With an incidence of 2,24 case per person year, they bring dehydration which are first causes of CDPS stay for observation (55,1%). We noticed an over consumption of antibiotics: 32,5% of the consultations ends up at their antibiotic medical prescription. Conclusions: This study identifies respiratory and digestive diseases as the main actors of the pediatric morbidity. Other spacio-temporal studies are necessary in order to understand associated factors. One-site personnel formation, reinforcement of medical facilities and development of prevention measures could foster the optimization of pediatric care in that isolated environment.
