Description épidémiologique et parcours de soins des enfants nécessitant une prise en charge en unité de soins intensifs en Guadeloupe de 2013 à 2015 : une étude rétrospective, descriptive, observationnelle

Abstract : Objectives: Study the characteristics and the care pathway of children requiring care in Intensive Care Unit (ICU) in Guadeloupe between 2013 and 2015. Materials and methods: Retrospective observational study including all children under 18 requiring care in ICU at the CHU Pointe-à-Pitre Abymes (CHUPPA), Guadeloupe, between January 1, 2013 and December 31, 2015. Demographic characteristics, reason for admission, and care pathway were analyzed for each child. Results: 223 children were included. The most common reasons for admission were acute respiratory distress before the age of 7 and trauma after the age of 7 (traffic collision and penetrating trauma respectively 14% and 16% of the overall sample). Mortality rate is 10% (22 children including 7 for trauma). Children were divided at the CHUPPA between the neonatal ICU (27%, including all infants under 3 months) and the adult ICU (68%); 10 children were directly transferred to pediatric ICU (Martinique or Metropole). The length of stay in ICU at CHUPPA is short. Forty children were transferred to pediatric ICU; 70% of the transferred children are intubated. Sickle cell disease is the third most common reason for hospitalization (12% of the sample), with 69% admitted for acute chest syndrome (ACS) including 15% who died (100% from ACS). Half of the children admitted for sickle cell disease were intubated. Conclusion: This study reveals the necessity to create a pediatric surveillance unit in Guadeloupe to optimize children’s care pathway.
