Conciliation des traitements médicamenteux : expérimentation du rapport Med'Rec 2015 de l'HAS dans un établissement psychiatrique

Abstract : Medication reconciliation is a method aiming at securing the drug therapy of patients as it ensures the prescriptions continuity during transitions between town and hospital. The experiment report Med'Rec 2015 from HAS proposes some tools for standardization of practices. Are the recommendations made adapted to our psychiatric institution and more generally, to psychiatry? We have conducted a test phase of 3 months in a pilot unit. Out of 88 eligible patients (all admissions), 67 patients received a medication reconciliation. The average age was 44,9 years. A patient interview was conducted in 91% of cases, and therefore was not an obstacle to medication reconciliation. 46.3% of the patients didn’t receive a medication reconciliation within the 24 hours; the psychiatric pathology was the cause of the delay in the medication reconciliation process in 50% of cases. The “Unintentional Discrepancies/Patient” indicator is 0.33; it is lower if we focus on patients over 65 years: 0.14; and higher for patients having at least one somatic drug on medication list at admission: 0.61. In addition, 91% of unintentional discrepancies concern somatic drugs and 59% are omissions. The most common medication errors are omission of somatic drugs that illustrates the problem of the border between psychiatric and somatic care approach. The new criteria "patients with at least one somatic drug on medication list at admission" will allow targeting the patients at risk regardless of their age to fit an average young age. We did not highlight any brake to the CTM linked to Psychiatry but some challenges that may be managed by adapting our organization.
