Description de la prise en charge médicale et de la prévention du VIH en Polynésie française, de la situation épidémiologique par rapport aux objectifs de l’Organisation Mondiale de la Santé et analyse des facteurs influençant l’efficacité du traitement

Abstract : Objectives: the objective of this study is to describe the overall situation of HIV in 2015 in French Polynesia (FP) at curative and preventive level, and to analyze the factors associated with virological success in patients. Methods: variables were collected in January 2016 in the active file of HIV patients in FP, and the data at the pharmacy of the hospital Taaone (Tahiti). Analyses were performed to highlight the factors influencing the virological success of a patient under treatment. Interviews were conducted with different actors in HIV to describe the treatment cascade in Pf. Results: the active file of HIV in 2015 in FP concerns 131 patients, including 8 cases declared in the year. 52.2% of them were born in Pf, 61.1% are male, 74.8% of them live in Tahiti. The prevalence of detected cases is variable according to the islands. Transmission is primarily through sexual contact (87%) with 39% of homosexual contamination. The HIV prevention has to be strengthened (prevention message, events, health regulations, promotion of condoms). Screening services are in a positive dynamic (increased screening centres and targeted screening, provision of self-tests). By contrast, there are weaknesses in support of the patient (lack of therapeutic education and no patient association). 91.8% of followed-up patients are treated, of which 79.5% had an undectable viral load (VL). Virological success is associated with a simple treatment, the fact of living in Tahiti, to be a non-smoker, or being an expatriate. Conclusion: this work allows to highlight problems of HIV in FP and suggests corrective measures at various levels.
