Bilan de l’activité du plateau technique obstétrical d’Annecy de Juin 2013 à décembre 2016

Abstract : Context: In order to reach the stage of "becoming parents", some couples wish to establish, before the birth, a true link with the person who will accompany them throughout the pregnancy. Three liberal midwives offer comprehensive support at birth as well as a physiological delivery at the Annecy Hospital Center as part of a technical support. The PT is the rental of premises, equipment and personnel necessary for the practice of the birth by the liberal midwife who manages it then in full autonomy and under its own responsibility. It allows the possibility of an immediate transfer in case of obstetric complications to the medical team Objectives: To determine the evolution of the activity of the PT of Annecy from January 2013 to December 2016 and the inventory of requests for access and deliveries made on the PT. METHOD: Retrospective observational epidemiological study of the 210 complete applications for deliveries in PT from June 2013 to December 2016. Results: PT access demand increased from about 4 / month in 2013 to 7 / month in 2016. Of the 210 PT deliveries analyzed, 23% were transfers during pregnancy, 12% Of transfers during work (13.4% trigger, 11% request for epidural analgesia), and 6% postpartum. Primiparous have an increased transfer risk (28% vs 30%) in multiparous (p = 0.03). The delivery occurred at home for 8% of women. SF practices respect physiology (15% artificial rupture of membranes, 6.7% episiotomy). The breastfeeding rate is much higher than the national rate (95%). No re-hospitalization took place after the early return home which takes place within 4 hours after birth. Conclusion: Midwives are increasingly being asked to support couples in their demand for natural and demedicalized childbirth. Their role is to be the least interventionist while respecting the indications of transfer if necessary. An expansion of indications to balanced gestational diabetes is also planned.
