Dépistage de la consommation de substances psychoactives pendant la grossesse : état des lieux des pratiques et des freins

Abstract : AIM : To establish the prevalence of a systematic screening of the use of psychoactive substances during the pregnancy first consultation, and identify the representations as well as the difficulties encountered by midwives for this screening. METHOD : Descriptive study carried out using self-surveys in november 2016 to midwives from consultation services in maternity wards and to private midwives from the Drôme, Isère, Savoie and Haute-Savoie departments. The evaluation criteria were the percentage of professionals who declare approaching the subject during prenatal consultations, the terms mentioned by midwives regarding the screening, the level of screenings declared for each substance (tobacco, alcohol, cannabis) and the difficulties expressed. RESULTS : 119 midwives have been included and 2 excluded. 43.7% of them declared systematically approaching the consumption of tobacco, alcohol and cannabis during the pregnancy first consultation. The level of screening is of 82% for tobacco, 63% for alcohol and 45% for cannabis. 2.5% of midwives mention difficulties approaching the consumption of tobacco, 38.7% for alcohol and 37.8% for cannabis. CONCLUSION : The difficulties to approach the subject of psychoactive substances seems to impact the systematic realization of their screening. Before the heavy consequences caused by their consumption during pregnancy, it seems necessary to develop tools and to improve the training of professionals so as to reinforce their screening.
