Création d'une fiche d'information à l'usage des médecins généralistes pour réaliser une demande de télé-expertise en dermatologie, concernant les cancers cutanés

Abstract : Context: In France, cutaneous cancer incidence rises while general practitioners (GP) and dermatologist’s (DE) demography decreases. Tele expertise is a leading solution to answers the increasing lack of quick DE’s opinion. Despite the growing number of initiatives, there is no standardized short form for information exchange between DE and GP about patient and their diseases. This study aimed to define the exhaustive list of clinical, biological and therapeutic items that GPs had to specify when asking for a DE’s advice through a tele-expertise procedure regarding a suspicion of cutaneous cancer. The secondary objective was to propose a standardized form to use. Method: A qualitative study using a modified nominal group consensus technique was performed. Experts were DE and GPs in equal numbers. The study researchers recorded and facilitated the entire session. Items were sorted through (a 2 dimensions and 4 levels scale voting process ) the placards technique. The final list of items was structured in a pdf short form format by the researchers and a computer technician. The form was submitted to the experts for approval. This work didn’t require any specific ethical approval. Results: The expert group of 3 GPs and 3 DEs kept 26 items for which they added 2 photos of the cutaneous lesion. Experts unanimously approved the pdf short form. Discussion: This is the first oncodermatology French tele-expertise short form issued from a consensus technique. One of the limitations was the lack of expertise of the researchers to animate a nominal group. This could have decreased the quality of the procedure. The other limitation was that the expert group excluded 2 items through discussion without resorting to a vote. This is not allowed in such a consensus method, however as there was a clear consensus among the experts it reduces the impact of this limitation. The form needs to be evaluated for psychometric proprieties and users satisfaction. Lastly the short form will be compared to a simple tele-expertise request.
