Angioedèmes histaminiques versus bradykiniques : mortalité par asphyxie

Introduction: Isolated head and neck angioedema (AE) can have one of two major etiologic mechanisms: bradykinic, due to a congenital anomaly or acquired defective bradykinin metabolism, or histaminic due to specific (allergic) or non-specific mast cell degranulation. The objective of our study was to determine angioedema mortality and which etiology was most frequent in cases of death. Methods: We sought all cases of death by isolated asphyxiating AE reported in France between 2000 and 2014 via death certificates and/or the national pharmacovigilance database. The cases were evaluated and classified according to the most likely etiological mechanism: histaminic, bradykinic, or indeterminate when neither of the two etiologies was distinguishable. Angioedemas associated with severe anaphylaxis (anaphylactic shock, asthma) were excluded. Results: 1090 cases of death with a mention of angioedema occurring between 2000 and 2014 were analyzed: 888 from the national database of the Center for Epidemiology of Deaths (CépiDc) and 202 from the national pharmacovigilance database. Angioedema was given as the cause of death by asphyxia in 342 cases. The overall mortality by asphyxiating angioedema, for all causes, was estimated at 0.36 / million inhabitants (0.23 - 0.54). A bradykinic etiology (BkAE) was retained in 129 cases with a death rate per million inhabitants of 0.14 (95%CI: 0.06 - 0.26); and lethality of 0.27 per thousand patients per year (95%CI: 0.12 - 0.51). A histaminic etiology (HiAE) was retained in 96 cases (93 from the CépiDC database and 3 from the pharmacovigilance database), i.e. a death rate per million inhabitants at 0.09 (95%CI: 0.03 - 0.21); and lethality of 0.006 per thousand patients with HiAE per year (95%CI: 0.002-0.013). 117 cases of death by angioedema could not be classified to one or other physiopathological mechanism. Conclusion: This is the first national study of cases of fatal head and neck angioedema. Between 2000 and 2014 the predominant cause of death by angioedema was bradykinic. Given the very low prevalence of BkAE compared to HiAE, we found a 45 times higher risk of death when the angioedema was bradykinic than when it was histaminic. All HiAE appear to be related to an allergic mechanism. Consequently, particularly severe episodes must be initially considered as bradykinic and quickly reassess any first-line treatment that is inappropriate. A revision of the International Statistical Classification of Diseases (ICD) sed for death certificates is planned for 2018. Given the advances in knowledge over the last 30 years, a more detailed classification of angioedema based on the physiopathology is needed. This would improve the internal validity of future studies on death from angioedema.
Introduction : Il existe 2 grands mécanismes étiologiques des angiœdèmes (AE) isolés cervico-faciaux : bradykinique et histaminique. Leur prise en charge thérapeutique est radicalement différente. Certains AE peuvent être fatals par atteinte des voies respiratoires supérieures. L’objectif de notre étude était d'approcher la mortalité par AE et de determiner si un mécanisme prédominait dans les cas de décès. Méthode : Nous avons colligé les cas de décès par AE isolés asphyxiants déclarés en France entre 2000 et 2014 via les certificats de décès et la base nationale de la pharmacovigilance. Les cas ont été expertisés et classés selon le mécanisme étiologique qui paraissait le plus probable : histaminique, bradykinique, indéterminé. Les AE associés à une anaphylaxie sévère étaient exclus. Résultats : 342 décès par AE asphyxiant ont été retenus, établissant le taux de décès spécifique moyen par AE en France entre 2000 et 2014 à 0.36/millions d’habitants/an (IC 95: 0.23-0.53). 129 cas étaient bradykiniques (AEB), 96 histaminiques (AEHi) et 117 cas restaient indéterminés (AEI). Le risque de mortalité par AE était ainsi évalué à 0.14 décès/million d’habitants pour l’AEB (IC 95: 0,06-0.26), et à 0.09 décès/million d’habitants pour l’AEHi (IC 95: 0.03-0.21). Si l’on se rapporte aux prévalences très inférieures des AEB, la létalité par AEB est 45 fois supérieure à celle de l'AEHi. Discussion : 1ère étude nationale des cas d’AE isolés cervico-faciaux mortels, confirmant la particulière gravité des AEB. Les décès par AEHi semblent tous d'origine allergique. L'évolution de la Classification Internationale des Maladies (CIM 11, 2018) permettra d'affiner ces données à l'avenir.
