Approche biologique et psychométrique du syndrome d’épuisement professionnel : étude cas-témoins comparant 54 auditeurs financiers en burnout versus 86 témoins

Abstract : Introduction: the study of burnout syndrome (BOS) is complex. The scientific literature shows that biopsychométric approach is interesting. The objective is to study the differences in blood composition in the financial auditors suffering of BOS. Method: we performed a case-control study including 54 symptomatic employees (30 Male, 24 Female mean age 31.7 years ± 7.4) in comparaison to 86 control patients (41 Male, 44 Female, mean age 30.8 years ± 7.1). The subjects were evaluated by a physical examination, a selfquestionnaire on sleep, and psychometric scales: Karasek, Maslach Burn out Inventory (MBI), Hospital Anxiety and Depression Scale (HADS). An intravenal blood test was taken for each subject. Parameters measured are hemoglobin glycosylated (HbA1c), glycemia, cholesterol, triglycerides, protein C-reactive, thyroid stimulating hormone, 25- hydroxyvitamin D3 (25(OH)D3) and blood count. The variables were compared by the test of X² et de Fisher. Results: the HbA1c and insomnia (DSM V criteria) are significantly increased in the BOS group (4.66 ± 0.57 versus 3.0 ± 0.51, p < 0.001), (OR = 21.5 [8.8 - 52.3], p < 0.001). The 25(OH)D3 is decreased in the BOS group (17.7 ± 6.9 versus 28.9 ± 4.9, p < 0.001). Conclusion: HbA1c, 25(OH)D3 and insomnia were significantly associated with burnout. These parameters could be useful in screening and monitoring of employee burnout. Further studies are needed to confirm these results.
