The influenza epidemic in 2015-2016 : late character, clinical severity and emergence of the B Victoria virus

Résumé

Objectives : to describe the epidemiological, clinical and virological characteristics of each patient with a laboratory proven influenza during the epidemic in 2015-2016 at Bordeaux University Hospital. Methods : this retrospective study included each patient aged more than15 years and 3 months, with an influenza positive RT-PCR during a consultation or an hospitalisation from september 2015 to may 2016. Results : a total of 110 patients were included, with a sex-ratio of 1,34 for men. The mean age was 53,3. One hundred and two patients (90 %) had at least one risk factor for a complication and/or a severe influenza infection. Sixty-two patients (56 %) were hospitalized, 40 (36 %) were admitted in intensive care unit. Thirty four of these patients (85 %) needed mechanical ventilation or non-invasive ventilation. The strains analysis showed that 50 (45 %) patients were infected with A(H1N1) virus, two (2 %) with A(H3N2), 47 (43 %) with B/Victoria virus and there were only one infection with the B/Yamagata virus. Twenty five patients (23 %) were vaccinated. Concerning these patients, 12 (48 %) and 13 (52 %) had influenza caused by a B/Victoria virus and A(H1N1) virus, respectively. The epidemic peak was late with 93 cases (85 %) between march and may. Conclusion : this study confirms the severity of influenza infection. The influenza B/Victoria virus represented almost half of the cases. There was a mismatch between the B/Yamagata strain contained in the 2015-2016 vaccine and the B/Victoria circulating strain.
Objectifs : décrire les caractéristiques épidémiologiques, cliniques et virologiques des patients ayant eu un prélèvement positif pour la grippe au Centre Hospitalier Universitaire de Bordeaux durant l’épidémie 2015-2016. Matériel et méthodes : cette étude rétrospective a inclus tous les patients de plus de 15 ans et 3 mois avec une PCR grippe positive lors d'une consultation ou d'une hospitalisation, du 01/09/2015 au 31/05/2016 au Centre Hospitalier Universitaire de Bordeaux. Résultats : au total, 110 patients ont été inclus avec un sex-ratio homme/femme de 1,34. L’âge moyen était de 53,3 ans. Cent deux patients (90 %) avaient au moins un facteur de risque de complications et/ou de grippe grave. Soixante-deux patients (56%) ont été hospitalisés, 40 (36 %) ont été admis en réanimation. Trente-quatre de ces patients (85 %) ont nécessité une ventilation invasive ou non-invasive. L’analyse des souches a montré un virus A(H1N1) pour 50 patients (45 %), A(H3N2) pour deux (2%), le virus B/Victoria pour 47 patients (43 %), et une seule infection par le B/Yamagata. Vingt cinq patients (23 %) étaient vaccinés. Chez ces patients, 12 (48 %) ont présenté une grippe B/Victoria et 13 (52 %) une grippe A(H1N1). Le pic épidémique a été tardif avec 93 cas (85 %) entre le mois de mars et de mai. Conclusion: cette étude confirme la sévérité de la grippe. Le virus B/Victoria a représenté presque la moitié des cas de grippe. Il y a eu une inadéquation entre la souche B/Yamagata contenue dans le vaccin 2015-2016 et la souche circulante B/Victoria.
