Influence du stage de deuxième cycle en médecine générale sur le choix aux ECN : étude réalisée à Bordeaux, Lille et Lyon en 2016

Abstract : Introduction : since the establishment of the general practice (GP) internship in the second cycle of medical course, studies have shown that the achievement of this internship positively influenced the choice to become a general practitioner, without verifying that this influence was achieved in the choice of specialty. The aim of this study was to determine the influence of the GP internship on the choice of the specialty «General Practice» after the French pre-residency program examination of 2016. A secondary objective is to find out if the ranking of students who choose GP is better among those who have done this internship. Method : prospective, multicenter cohort study, online questionnaire for students in the final year of graduate studies (DFASM3) by e-mail and social networks in early 2016, before the pre-residency program examination. Collection of nominative data allowing to know classification and choice of specialty as described in the Official Journal in October 2016. Results : we surveyed the DFASM 3 of three medical schools (Bordeaux, Lille 2 and Lyon Est), ie 1366 students and obtained 525 answers (38.4%). After applying the exclusion criteria, 471 students were affected after the procedure of choice. 272 students (57.75%) had completed the GP internship. 207 students out of 471 chose the specialty of GP (43,95% compared to 44.85% at the national level). Those who had completed the internship in GP during their studies wanted significantly more to become GP (50% against 35.7%, p = 0.0025). GP’s future residents who had completed the internship were significantly better ranked (average ranking 4616 vs 5319, p = 0.016). Discussion : our population from three faculties is representative of the 2016 French pre-residency program class. The internship has a strong positive impact in favor of General Practice: offering a valuable internship for all students will improve engagement in general practice.
