Vécu, motivations et ressentis des patientes consultant un médecin généraliste en cabinet de ville pour la pose d’un dispositif intra-utérin (DIU)

Abstract : Introduction: in France, since the legalization of abortion in 1976 and the medicalization of contraception at the same time, the number of abortions has not diminished and hardly the one of unforeseen pregnancies, hardly. Yet France is pleased to have 90% contraceptive coverage. It appears obvious that the abusive use of pills, with a real mediocre Pearl index, at the expense of long-term means of action such as the IUD, more effective, does not allow to control the onset of pregnancies. However, these means of contraception requires an experienced operator, gynecologists are increasingly inaccessible and few general practitioners are involved. Objective: We have tried to understand the determinants that encourage women to be put in an IUD at the general practitioner. This can be an easy access solution to this contraceptive method. Method: 11 women, in semi-rural areas, of the canton of Sud-Gironde were interviewed individually in semi-structured interviews. The meetings were recorded by a double audio technique and were manually transcribed. Results: Women have expressed a sense of confusion and loneliness in the management of their contraception. They have highlighted brakes and reluctance to consult gynecologists in general. They have progressed, particularly in handling their IUDs with the help of the general practitioner. They find this operator more suitable because there is a strong relationship of trust between them. Moreover, they find it, more available, more attentive and are satisfied to have to do to a single examiner, for the establishment of this contraceptive that generates modesty. They are satisfied with their skills and would like this practice to be generalized to all general practitioners. Conclusion: By breaking down some barriers to IUD access, such as being able to access it more naturally at a general practitioner, this would promote a contraceptive device with a real effectiveness much better than the one currently in place.
