Création d’un banc de test des humidificateurs chauffants en ventilation invasive

Abstract : Introduction: gas humidification is essential during invasive ventilation as upper airways are by-passed and can no longer provide sufficient heat and moisture in order to avoid some serious complications such as tube occlusion. In case of flow rate increase as per example with leaks (intentional or not), humidification provided by heated humidifiers can be altered. The aim of this study is to evaluate through a bench test the quality of humidification of heated humidifiers in different conditions of ventilation. Methods: we tested four heated humidifiers in eight configurations: using a non-vented circuit with an exhalation valve or a vented circuit with an intentional leak, tidal volume of 600 or 1000 ml, with or without a non-intentional leak. The absolute humidity (AH) was measured at the Y-piece just before the artificial lungs placed in a climatic chamber set at 34°C in order to simulate the body temperature. Results: with the non-vented circuit, AH is always measured above recommendations of 33 mg/l (from 33 mg/l [30-33] to 40 mg/l [40-41]). With the vented circuit, for tidal volume of 600 ml and without non-intentional leak, AH is measured between 35 mg/l [35-35] and 39 mg/l [39-39]. For tidal volume of 1000 ml and/or in case of non-intentional leak, two humidifiers show a decrease in AH below recommendations. Conclusion: this bench test shows that under certain conditions of ventilation such as high tidal volume or in case of leaks, some heated humidifiers do not allow sufficient humidification and can endanger the patient.
