Percevoir l'incertitude de son médecin généraliste en consultation : analyse interprétative phénoménologique de vécus de patients

Abstract : Context: The general practioner’s uncertainty has often been studied from the professional’s perspective, seldom from patients’. Objective: To explore the experiences of patients who had perceived their general practitioner’s uncertainty in ambulatory consultation. Method: A qualitative study was built with patients from Isère, Savoie and Haute-Savoie, interested in the subject and with willingness to participate. Following 6 semi-structured individual interviews that were held from November 2016 to July 2017, an interpretative phenomenological analysis was undertaken by 2 interviewers. Results: Two women and 4 men, aged 30 to 78, were interviewed. Four super-ordinate themes were identified: “instant comparison between facts and one’s bearings: either a break or continuity”, “the need to cope with disappointment in one’s fallible physician”, “the need for the physician to make one feel safe : focus on the physician”, “taking of a role : towards autonomy”. Patients in a recent relationship and naive from this kind of experiences with their physician instinctively refused this fallible physician. They wondered about their physician’s credibility and adjusted their expectations of him. Getting vulnerable, they searched their physician, for signs of his protection. Patients whose trust had been built had an easier acceptance. Finally, all of them took a role in the relationship with their uncertain physician. Conclusion: The patient's experience has been through a dynamic, that led him from an instinctive rejection to some satisfaction; from vulnerability towards autonomy. Beyond certainties, patients especially expected their physician to give his benevolent protection.
