Métastases hépatiques dans le cancer colo-rectal : faut-il différencier les synchrones des métachrones ? Étude des critères pronostiques onco-chirurgicaux sur une série rétrospective grenobloise de 178 patients

Abstract : Introduction : Colorectal cancer is the third cancer in Europa in terms of incidence. Liver metastases, synchronous in 20 to 30% of patients or metachronous in the follow up in 50% of the patients, impact the prognosis. Methods : One hundred and seventy eight patients treated by surgery and/or radiofrequency for liver metastases exclusively have been included. Clinical, radiological data and features of the primary tumor and metastasis have been analysed. Results : The median survival was 52,5 months in the synchronous population and 70,2 months in the metachronous population. The initial lymph node staging and preoperative radiofrequency (p=0,03 and 0,04 respectively) are associated with a decrease of the overall survival in the synchronous population. Multivariate analysis indicates that a number of metastasis > 5 and delivery of post operative chemotherapy are significantly associated with decreased overall survival in case of metachronous metastasis. Conclusion : Synchronous presentation is a pejorative feature correlated with a OS decreasing. Synchronous and metachronous liver metastasis don’t share the same prognosis features. The onco surgical management should be differenciated. Metachronous metastasis can be cured by an exclusive surgery but our study demonstrates the benefits of an adjuvant treatment.
