Boucle Vélocité / Pression Aortique (PU Loop) pour l’évaluation continue de la post-charge ventriculaire des patients sous anesthésie générale

Abstract : During general anesthesia (GA), cardiovascular homeostasis can be altered. While vasopressor therapies can be used to treat GA induced arterial hypotension, acting by elevating cardiac afterload, no afterload monitoring is currently available. This study aimed at describing GALA (Global AfterLoad Angle), a Velocity-Pressure loop (PU loop) derived parameter, during constitutive and pharmacological alterations of cardiac afterload. Twenty patients were grouped into high or low cardiovascular risk and analyzed. While mean arterial pressure (MAP) was equivalent, cardiac afterload was higher in high risk patients, so was GALA (68±6° vs 41±15° ; p<0.001). A positive association was found between GALA and each cardiac afterload parameter. During vasopressor administration, we observed a rise in MAP, a decrease in cardiac output and an increase in cardiac afterload (p <0.001). GALA was also significantly higher after vasopressors (p < 0.001). A differential effect of vasopressor was shown by GALA and by cardiac afterload parameters. GALA could represent a pertinent marker of cardiac afterload during GA, and could be used as a continuous vasopressors’ effects monitoring in operating theatre, in order to improve patients’ safety.
