Étude prospective sur la gestion péri-opératoire des anticoagulants oraux direct chez les patients subissant une intervention de chirurgie orale. 1ère partie : revue de la littérature et mise en place du protocole d'étude

Abstract : Direct oral anticoagulants (DOAC) are part of the current therapeutic arsenal of trombo-embolic disease and present numerous advantages towards anti-vitamin K. However, due to their recent occurrence, the consequence of their use in oral surgery, especially on the hemorrhagic risk, is still unknown and little documented. The main objective of this work is to propose a study protocol in order to evaluate the rate of thromboembolic and hemorrhagic events occurring during the perioperative period in patients treated with long-term DOAC, and benefiting from an intervention of oral surgery. The first part of this thesis reviews the trombo-embolic diseases, at venous and arterial level. The second part deals specifically with DOAC and their places in the therapeutic arsenal. The third part presents the results of a review of the literature on the hemorrhagic risk of DOAC patients during an oral surgery. Finally, the last part describes the protocol set up for this prospective, multicentre, observational and non-interventional cohort study.
