Attitudes and practice related to Helicobacter pylori infection among primary care physicians in Guadeloupe, French West Indies

Résumé

Helicobacter pylori(Hp) is a well-known worldwide bacterium promoting gastric ulcer and cancer. It’s resistance to standard triple therapy used since 1999rose and the screening indications enlarged to primary cares. Consequently, France modified its guidelines in 2015, and this study aimed at evaluating whether the primary care physicians in Guadeloupe, a small island in the French west indies, managed to apply these recommendations, from july to October 2016.117 questionnaires were analysed, representing 42% of the general practitioners. 97% had patients positive for Hp. Less than a third knew they should seek for the bacterium before long term treating with a non-steroidal anti-inflammatory, aspirin, or proton pump inhibitor, and in front of an anemia due to iron or vitamin B12 deficiency without obvious cause. 13% still used the Hp serology as a diagnostic test. 16% used the bismuth/non-bismuth containing quadruple therapy and 57% the sequential therapy, 18% still used the standard triple therapy as a first regimen. A third didn’t control the effective eradication of the bacterium. Two third had at least one treatment failure, then their treatment choices are the quadruple therapy for half of them and the sequential regimen for 16% of them. At least, 77% send their patients to the gastroenterologist for diagnostic endoscopy and antibiogram. 62% said they received a protocol by the gastroenterologist. Statistical analysis didn’t find any correlation between any first regimen prescribed and observed therapy failure. These results can be explained by the less than a year recommendations which probably didn’t allow the physicians to adapt, especially considering that the official synthetic guidelines were published after the survey. One can think updating primary cares physicians practices can improve significatively Hp eradication.
Helicobacter pylori (Hp) est une bactérie ubiquitaire de l’estomac humain, sa pathogénicité dans les gastrites, ulcères et cancer gastrique est bien identifiée. L’émergence de résistances et l’extension des indications de dépistage et de suivi nécessite un élargissement aux soins primaires. Notre étude s’intéresse aux pratiques de prise en charge d’Hp par les médecins généralistes de Guadeloupe de juillet à octobre 2016 par rapport aux recommandations de 2015. 117 questionnaires ont été recueillis (taux de réponse 42%). 97% ont eu des patients avec Hp en 2016. Moins du tiers recherchait Hp avant un traitement au long cours par anti-inflammatoires non stéroïdiens, aspirine et inhibiteur de la pompe à proton et devant une anémie par carence en fer ou en B12 sans étiologie retrouvée. 13% utilisaient encore la sérologie Hp comme test diagnostique. Le traitement de première intention consistait pour 16% à une quadrithérapie, et 57% à une thérapie séquentielle. 18% utilisaient encore la trithérapie. Un tiers ne pratiquait pas toujours un contrôle d’éradication. Deux tiers avaient observé au moins un échec thérapeutique dans l’année. Le traitement de seconde intention était la quadrithérapie dans la moitié des cas, la thérapie séquentielle dans 16% des cas. En troisième intention, 77% adressaient bien pour fibroscopie et antibiogramme. 62% déclaraient recevoir un protocole du gastroentérologue. L’analyse statistique ne retrouvait pas de corrélation entre la survenue d’un échec thérapeutique et un type de traitement de première intention. Ces résultats peuvent être expliqués par le caractère récent des recommandations (2015) n’ayant peut-être pas permis aux médecins de s’adapter. La fiche officielle de prise en charge a été publiée a posteriori de l’enquête et on peut penser que le bénéfice d’une mise à jour des pratiques sera important.
