Revue de sortie d’aplasie post intensification thérapeutique des patients atteints de myélome multiple : autogreffes depuis 7 ans au CHU de Grenoble

Abstract : We retrospectively analyzed a total of 188 autologous stem cell transplantations (ASCT) performed in patients with multiple myeloma (MM), at Grenoble Hospital, between January 2010 and April 2017, in order to evaluate clinical activity and identify factors affecting hematopoietic recovery. In the univariate analysis, age, gender, MM type, disease status, CD34+ cell doses before or after cryopreservation, transfusion requirement were not statistically significant, on time to neutrophil engraftment. Only 2 factors, patients not included in clinical assays (p=0,009) and mobilization modalities with G-CSF alone (p=0,034) are statistically significant and prognostic factors for early engraftment. Our analyses confirm previous findings that patients may be mobilized with G-CSF alone, so we propose this mobilization regimen for patients not included in clinical assay. We also demonstrate that a CD34+ cell dose > 5 x 106 before cryopreservation, is required to reduce transfusion requirement (p=0,006). Our study showed a linear correlation between hematopoietic recovery and CD34+ cell dose infused after cryopreservation but more studies are required to confirm this point.
