Caractéristiques épidémiologiques des PVVIH suivies au Centre Hospitalier de Mayotte (CHM) du 01/01/2014 au 31/12/2015

Abstract : Background and Aims: Mayotte is a French island located in the Indian Ocean and surrounded by areas of high HIV-prevalence. Socio-economics conditions are deteriorated and population movements are important, leading to the spread of the epidemic. The small number of available publications prompted us to carry out this study based on the epidemiological characteristics of PLWH followed up in Mayotte. Methods: this is a retrospective and descriptive epidemiological study on the 01/01/2014 and a prospectively epidemiological study until the 31/12/2015, based on the medical files of HIV-positive patients followed up at the Hospital Center of Mayotte. Results: 212 PLWH were included. The studied population is predominantly female. The median age is 37 years, with a quarter of newly diagnosed people aged between 15-24 years. PLWH were born mainly in the Comoros (47%). Men are four times more likely to be at the AIDS stage (OR = 3.63, IC95 [1.77 - 7.45]). Precariousness is important with 48% of PLWH being unemployed and 30% without social security. Only 63% of treated patients have an undetectable viral load. People without medical coverage are almost four times more likely to be uncontrolled (OR=3.43, IC95 [1.86 - 6.31]). Conclusion: PLWH followed up in Mayotte present the characteristics of the African epidemic. While routine screening and early treatment of pregnant women are well organized, epidemic is less well controlled than in metropolitan France with only two-thirds of undetectable patients and vulnerable subgroups: young people, people in precarious situations and men who have an increased risk of reaching the AIDS stage. Screening efforts, particularly through rapid diagnostic orientation tests, should help to better target these populations and limit people lost to follow-up. The others major public health issues are health education and the limitation of risky practices.
