Mise en place d’un projet d’éducation thérapeutique concernant l’amblyopie au sein du service d’ophtalmologie du CHU Gabriel Montpied de Clermont-Ferrand

Abstract : Purpose: Amblyopia, a developmental pathology of the visual integrator (primary visual cortex), mainly expresses itself by an impairment of visual acuity. This is an asymmetrical phenomenon in most cases, we call it monocular visual impairment. This pathology is caused by an abnormal visual experience and occurs in the first years of life. It is the most common cause of children’s poor unilateral vision. Its treatment, effective as long as the cerebral plasticity is optimal, is based on the prescription of the total optical correction and on the occlusion of the healthy eye. These are visible medical devices that can make treatment difficult and unpleasant for the children and their families. During my internships, I was confronted with amblyopic patients’ difficulties. Objectives and method: The aim of the dissertation was to set up a therapeutic education project, concerning amblyopia and its treatment in the ophthalmology unit of the CHU Gabriel-Montpied in Clermont-Ferrand, in order to help patients understand the pathology, to accept it better and thus tolerate the treatment better. To do so, I have used a clinical study carried out in 2015 within the unit, dealing with the difficulties of treatment and the lack of therapeutic compliance concerning this pathology. The patients and professionals that I met throughout my study, thanks to their experiences and their findings, allowed me to enrich my approach. Results: The development of this project has led me to the creation of an educational booklet concerning amblyopia and its treatment, for children and parents. It explains with simple words and fun way the normal processes of visual development, the progression of the pathology and its consequences, the treatment to cure it, and the follow-up that it requires. This educational resource will enable the child and parents to visualize the pathology, understand the value of the treatment, and realize that they are major actors in this treatment. It will also support the healthcare team when they give explanation during the consultations and thus facilitate with the care-givers relationship. Discussion and conclusion: Studies in other countries have demonstrated the effectiveness of therapeutic education about Amblyopia. Furthermore, it has proved that this approach is effective for many chronic diseases. Therapeutic education is central in medical issues, it seems to me appropriate to support this project and share it with as many people as possible. This should help patients and their families as well as professionals.
