Évaluation de l’utilité et acceptabilité en médecine générale en Haute-Normandie, d’un outil internet d’aide au dépistage et à la prise en charge des troubles du comportement alimentaire, intégrant le questionnaire SCOFF-F

Abstract : Eating disorders (EDs) have been steadily increasing in recent years and are still underdiagnosed. The DSM-V criteria for the diagnosis of ED have been changed to broader categories: Anorexia Nervosa and Behaviorally Similar Disorders (AN-BSD), Boulimia Nervosa and Behaviorally Similar Disorders (BN-B5D) , Binge-eating disorder and BehaviOrally Similar Disorders (BED-BSD) and Eating Disorders Not Otherwise Specified (EDNOS).
AIMS :
Our first objective was to create a diagnostic-facilitation website (Expaliim) with care advice, for broad ED categories, featuring the SCOFF-F questionnaire. Our second objective was to evaluate its feasibility among General Practitioners (GP) on general population.
METHODS :
GPs from Upper Normandy were contacted to test this website , either by mail , or by direct interview. They were asked to test it on of their patients, and then to fill up a questionnaire in order to evaluate their opinion on the relevance and applicability of the website. This study was conducted in 2016-2017.
RESULTS :
A third of GPs (31,2%) who were invited to test Expalim responded and filled up the final questionnaire. 78% of the respondents were most likely "to recommend it to a colleague'. 61% found it "Quite" or "Very" effective for diagnosis and 83% were "Satisfied" with the information provided to help them for further guidance of the patients. Similarly, 95% of the GPs appreciated the user-friendliness of the website. Two GP out of three originally contacted did not use the website.
CONCLUSIONS :
Although, Expalirm was found to be an appropriate means of screening for ED symptoms in the general population, GPs motivation for training in this type of e-health tool is limited by various barriers: lack of information technology skills, lack of medical knowledge, symptoms and diagnosis, and lack of time during medical consultation. Beyond this first feasibility approach, further studies are required to validate this website .
