Représentations de la visite à domicile à La Réunion et facteurs d’influence : étude qualitative auprès de 12 jeunes médecins généralistes

Abstract : Background: as the medical density is being diminished, there is a real political wish to promote ambulatory care. In Reunion Island, because of the lack of hospital care and specialists, patients need more ambulatory care than in mainland. Young doctors are criticized for doing less home visits but have never been questioned about it. Objective: evaluate the representations of ambulatory visits by young general practitioners in Reunion Island and the influencing factors. Method: qualitative study with semi-structured interviews of 12 young general practitioners in Reunion Island. Three main themes were developed in the interview guide: Study of the perceptions of the ambulatory visit, answers to the elders’ critics and proposals for the future of ambulatory visits. Results: the visit is known to be essential but is considered as an activity in itself that requires time, courage and organization. Their perception is largely influenced by the availability of help like nurses, the kind of patients they have and the aura they wish to have within the society and in their families. Aware of the choice they made, general practitioners justified it by the quality of care to provide within an already overbooked agenda while keeping vital time for them. The local population was, however, described as more respectful than in mainland which is one of the positive sides of ambulatory visits. Conclusion: the perceptions of ambulatory visits by young general practitioners seems to be the same as their elders. They are choosing their practice according to the described influence factors. The idea of transferring knowledge to trained paramedics could be a way to give more satisfaction about this practice as well as creating a more sustainable way of conducting visits that are more important than ever for patients and the authorities.
