Évaluation du dispositif des Consultations Non Programmées assurées par les médecins généralistes au Service d’Accueil des Urgences du Centre Hospitalier de Périgueux (24)

Abstract : Introduction: Non Programmed Consultations (NPCs) at the Emergency Department of the Hospital Center (HC) of Périgueux were entrusted to the general practitioners in town during the hours of Permanence of Care (PC). The aim is to respond to the constantly increasing activity in the emergencies. Methods: this retrospective, mono-centric, observational, descriptive and cross-sectional study was conducted between february 2017 and april 2017. The objective is to describe the activity of the general practitioners during NPCs and to determine the proportion of patients who could have benefited from this offer of care, among all those who consulted spontaneously in the emergency department during this period. Results: 1016 patients were included, and among them, 102 (10.04%) were reoriented from the hospital to the general practitioner providing NPCs. Among these 102 patients, 97 (95.1%) were able to return home without further examination or expert advice. The general practitioners’ activity was zero 22 times out of 62 guards (35.5% of the time). Over the same period, 154 patients were seen by the emergency department and returned to their homes without examination or expert advice. Trauma represents a large majority of their activity (62%). The ENT symptoms and rheumatology represent 10%. Conclusion: general practitioners' activity was below expectations. However, it can be assumed that by widening their scope, it is possible to improve this system. With adjustments, the NPCs of general practitioners in emergency departments can become one of the solutions that will respond to the overactivity observed in the emergency rooms of the HC Périgueux. It is estimated that 116 (+ 113,7%) is the number of additional patients who could have been seen at NPCs.
