Évaluation de l’impact d’un dispositif dédié de consultation d’hypnose médicale sur la qualité de vie de patients drépanocytaires adultes du centre de compétence régional, CHU de Bordeaux

Abstract : Introduction: the main goal of this study is to evaluate the impact of medical hypnosis consultations (MHC) on the quality of life (QOL) of adults patients with sickle cell disease (ASCD). The study was conducted in the Bordeaux University Hospital where the studied ASCD were treated. Methods: we carried out an observational study "before wards and after wards" by performing the assessment of the impact of MHC in the short and medium term, without a control group. The primary outcome measure (OM) was the QOL score (SF-36), the secondary OM was the Pittsburgh quality score, the patient’s satisfaction with MHC regarding personal goals and practice of self-hypnosis. The quantitative results of the scores were compared using the WILCOXON test applied to paired samples. Results: 18 ASCD were evaluated. These patients have a percentage of 89% with origins from sub-Saharan Africa, a sex ratio of 1 man for 2 women and an average age of 30 years old (19 to 66 years old). Qualitative approaches to ASCD satisfaction and quantitative changes in QOL and sleep suggest that MHC is associated with an overall and significant improvement in the QOL of the ASCD taking part in the study. The use of self-hypnosis was mostly acquired outside vaso-occlusive crises (VOC) and not during VOC. Conclusion: our study is the first in the world to evaluate the impact of hypnosis on all dimensions of ASP QOL using the SF-36. The MHC, integrating a general practitioner (GP), reinforces the significant place that the GP can have in the follow- up of the ASCD. Mixed studies of larger numbers with a control group may consolidate our results.
