Ressenti des médecins traitants de Rhône-Alpes concernant la prescription d’activité physique adaptée chez leurs patients

Résumé

Introduction: The lack of physical activity of the population is a public health problem. The new health law of January 26, 2016 introduced a "Sport by prescription" device. It allows patients with long-term illness to benefit from a prescription of adapted physical activity made by their general practitioner. The objective is to assess the feelings of Rhône-Alpes general practitioners regarding the prescription of physical and adapted activity for their patients. Material and method: Qualitative study by semi-directed interviews in the Rhône-Alpes region with general practitioners. Sampling was done at maximum variation, and interviews were conducted to data saturation. After triangulation, a thematic analysis was carried out. Results: Thirteen interviews were conducted between January and April 2017. The analysis of the results revealed three main themes. First, the place of the family doctor was discussed. They were aware that the patient's motivation was primordial. However, they did not feel competent enough, and thought that other health professionals would be better able to raise awareness among patients. Second, the creation of a network was important to them. It was supposed to be created to best support patients in their care, with the ultimate goal of empowerment. Finally, many obstacles are mentioned by the doctors: the lack of time for them and their patients, the need for a tedious financing search and a society that pushes patients into a dynamic of consumption. Conclusion: The regular practice of adapted physical activity can now be a public health tool but the modalities of implementation remain blurred. Despite the interest of the treating physicians to sensitize their patients to physical activity, they did not feel able to do so, and found many brakes that could jeopardize the long-term sustainability of this practice.
Introduction : Le manque d'activité physique de la population est un problème de santé publique. La nouvelle loi santé du 26 janvier 2016 a mis en place un dispositif « Sport sur ordonnance ». Elle permet aux patients en affection de longue durée de pouvoir bénéficier d’une prescription d’activité physique adaptée faite par leur médecin généraliste. L'objectif est d’évaluer le ressenti des médecins traitants de Rhône Alpes concernant la prescription d’activité physique et adaptée pour leurs patients. Matériel et méthode : Il s'agit d'une Étude qualitative par entretiens semi-dirigés dans la région Rhône Alpes avec des médecins traitants. L'échantillonnage s'est fait en variation maximale, et les entretiens ont été réalisés jusqu'à saturation des données. Après triangulation, une analyse thématique a été réalisée. Résultats : Treize entretiens ont été menés entre janvier et avril 2017. L'analyse des résultats a fait émerger trois grands thèmes. En premier lieu la place du médecin traitant était discutée. Ils avaient conscience que la motivation du patient était primordiale. Cependant, ils ne se sentaient pas suffisamment compétents, et pensaient que d'autres professionnels de santé seraient plus aptes à sensibiliser les patients. Ensuite, la création d'un réseau était importante aux yeux des médecins. Il devait être créé pour accompagner au mieux les patients dans leur prise en charge, dans un but final d'autonomisation. Enfin, de nombreux freins sont évoqués par les médecins : le manque de temps pour eux et leurs patients, la nécessité d'une recherche de financements fastidieuse et une société qui pousse les patients dans une dynamique de consommation.
