Étude descriptive et rétrospective sur l'impact de l'évaluation diagnostique effectuée par l'unité "Les Makaras" du centre hospitalier Valvert

Abstract : Problematic: The UMAA « Les Makaras » is a diagnostic unit for the diagnosis of autism spectrum disorders in adults in the PACA region in France. The subjects without intellectual disabilities are the most at risk of not being diagnosed, or being misdiagnosed with a pathology which is in fact only a comorbid disorder. But, studies show that diagnose these subjects improves their quality of life and allows a better adaptation of their medical care. In this context, we wished to carry out an original study concerning the subjects who had undergone a diagnostic assessment at the UMAA "Les Makaras" in 2015, to assess the impact at one year on a social, emotional and quality of life basis. Methods: 80 individuals without intellectual disabilities came to UMAA in 2015 and were included in our study conducted in 2016. A first mail explaining the reason for the study and the anonymity conditions for the questionnaire replies was sent to the subjects included in the study, followed by the questionnaire of 19 questions and a form of free and informed consent, sent by mail also. For individuals who did not respond one month after the sending of the questionnaire, only one phone call was made. Results: The study participation rate was 52,5 % with 42 subjects. The diagnostic assessment was negative for most participants (28 or 70%, n = 40) After the assessment, 50% of respondents (20, n = 40) said they had not been referred, 37,5% (15, n = 40) said they had been referred for psychiatric or psychological care, including 12,5% (5, n = 40) to cognitive and behavioral therapy. Only 24.3% of the participants (n = 37, 9) initiated treatment following the assessment. A minority of the participants perceived an improvement in their affective life (10 or 25%, n = 40) and in their social life (12 or 28,6%, n = 42) following assessment and eventually, the therapeutic management. Thus, the quality of life impact of the assessment was not positive for most respondents (30 or 71,4%, n = 42). Discussion: This study shows a little emotional and social impact of the diagnostic evaluation. This result must be qualified with the small delay between the assessment and the study which is only one year. In addition, a minority of participants initiated therapeutic management after the evaluation.
