Analyse rétrospective des patients porteurs d’anticorps anti-TIF1gamma au CHU de Bordeaux de novembre 2014 à février 2017

Abstract : Introduction: dermatomyositis are classified among autoimmune disease and the recent discovery of specific antibodies associated with dermatomyositis has profoundly changed our vision of this heterogeneous group of inflammatory myopathies. The hopes and expectations aroused by these antibodies were that clinical forms, prognosis and association to cancer could be predicted depending on the type of antibody detected. Among these newly discovered antibodies: the anti-TIF1Ɣ antibodies are commonly associated with a higher risk of neoplasia. Their routinely detection is now possible thanks to commercial kits based on immunodot method. Our study details the clinical and immunological characteristics of patients carrying anti-TIF1Ɣ antibodies. Methods: This single center French retrospective study was conducted from November 2014 to February 2017. All the patients who carried anti-TIF1Ɣ antibodies, detected by immunodot, were included. Results: total of 30 patients were included: 7 had an authentic dermatomyositis and 23 had been considered as false positive. Over the 7 dermatomyositis, all had cancer (precessional or diagnosed while caring) and at the end of compendium only one patient was alive. Over the 23 false positives, 18 patients had an autoimmune or inflammatory disease and all of them, except one, presented a significant dysimmunity. None of the false positives had cancer. Conclusion: DM with anti-TIF1Ɣ antibodies are severe. Cancer association is now well describe and related to poor prognosis. However, immunodot’s performances seem disrupted in case of autoimmune or inflammatory diseases or strong dysimmunity.
