Évaluations psycho-sociales des suspicions de violences sur mineurs : étude prospective menée au Centre d’Accueil en Urgence des Victimes d’Agression du Centre Hospitalier Universitaire de Bordeaux

Abstract : Violence against minors is an underestimated phenomenon with multiple consequences. Following the publication of a Psycho-Social Evaluation (PSE) methodology in the cases of suspicions of violence against minors, the authors aimed at focusing on the characteristics of children and families. Materials and methods: this is a prospective, monocentric study carried out from December 19, 2016 to March 31, 2017, at the Centre d’Accueil en Urgence des Victimes d’Agression (emergency unit for victims of assault) of the Bordeaux University Hospital. Children and families who benefited from PSE prior to any legal proceedings were included to the study. Results: 33 minors, 70% (n=23) of girls and 30% (n=10) of boys, mean age 7.6 years were received. They revealed spontaneously (n=17) single (51.5% ; n=17), sexual (63.6% ; n=21), physical (12.1% ; n=4) and psychological (6.1% ; n=2) violence and 18.2% (n=6) of the associated violence. 15 subjects (47%) presented psychological instability. Besides, our results show that in most cases, the perpetrator was a boy (79%, n = 26), minor (52%, n=17) who acted alone (87.9% ; n=29). Parents (44% ; n=8) and siblings (33% ; n=6) were involved in the family sphere. Parents, for 51.5% (n=17) divorced or separated, reported a personal history of abuse in 33% of the cases (n=11). Families were referred by a medical professional in a case on two, 21% (n=7) by the general practitioner. Following our assessment, a legal proceeding was retained for 22 cases (69%), 14 by a filing of complaints and 8 by a report filed to the public prosecutor of the Republic. Conclusion: subject to the size of our population, there does not appear to be any "profiles" of children and families likely to face suspicions of violence. The general practitioner in the front line should not remain alone but should refer to a specialist structure so that a multidisciplinary assessment is carried out in order to guarantee a child protection adapted to the reality of the danger.
