Accouchement en plateau technique versus prise en charge classique, en maternité de type I pour les grossesses à bas risque obstétrical : complications marterno-foetales et néonatales. Profil socio-démographique des parturientes

Abstract : Introduction: Does the technical support center combined with physiology and less medicalisation, generate a higher materno-foetal risk? Isn't the mother’s socio-demographic profile specific? Goals: To describe the maternal and neonatal complications during labour and delivery, as well as the parturients’ epidemiological profile depending on the medical care. Tools and Method: Troop's study including 291 low risks parturients. Maternal and perinatal data were analysed. Results: 85,71% of children were born after spontaneous vaginal delivery against 71,34% in level 1 maternity. This difference is not significant. It has been noted that within the technical support there were significantly lower rates of epidural analgesia (35,29% versus 69,73%), directed delivery (19,3% versus 87.25%), amniotomy (16,61% versus 47.87%), oxytocic used (16% versus 51.06%)and episiotomies (11,26% versus 25.44%). However there was no significant difference concerningthe neonatal exit. Although comparable from the geographical point of view, the parity and the corpulence, the parturients in technical unit were significantly older (64,08% were between 30 and 40 years old),with a higher level of education (82,44% have a higher education degree), consume less tobacco (15,53% only consume tobacco) and have a better social cover (6,67% of CMU). Discussion: Undeniably satisfied, the parturients accessing the technical unit enjoy a safe alternative and remain covered in terms of hospital care and expenses if necessary. Conclusion: The parturients who tend to go for the the technical support are on average older and belong to a higher socio professional category. This practice does not induce more maternal neither foetal complications and even improves the maternal prognosis.
